Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by and or crushing hazards, when the employer failed to ensure operators followed instructions outlined in the operations manual when parking or leaving a Caterpillar Front End Loader (Integrated Toolcarrier) unattended. This condition was observed on or about September 8, 2011, at the intersection of State Highway 332 and "This Way" street, when the employer failed to ensure employees engaged the parking brake, lowered all attachments to the ground, stopped the engine and removed the key while parkinig and or leaving the excavator unattended. This condition was observed, on or about September 8, 2011, at the intersection of State Highway 332 and "This Way" street, Lake Jackson, Texas 77566, the employer's procedures for properly parking a rubber-tired wheel front end loadeer were not followed by an operator. Among other methods, feasible and acceptable methods to abate this hazard includes, but is not limited to: a.Operators reading the Catepillar front end loader manual on topics such as machine operation,before operating the machine, and machine parking instructions. b.Employer ensure that operators do not leave front end loaders unattended with the power on and the engine running to go perform a different task. c.Retrain operators on the safe use and operation of the Catepillar front end loader, model IT-18F CAT. d.Park front end loaders more than 25 feet away employees work area. e.Maintain a continuing training program that inform operators of the hazards associated with operating front end loaders and earth moving vehicles. Pursuant to 29 C.F.R. 1903.19 within ten (10) calendar days of the abatement date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all operators do not leave front end loaders and earth moving vehicles unattended with the power on and the engine running.
