Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing-type hazards from inadequate blocking and bracing of heavy steel that was being being prepared for welding: (a)On or about May 18th, 2011, an employee was exposed to crushing-type injuries due to inadequate blocking and bracing of a heavy steel railroad spine car that had been cut in two, shortened, then tack welded back together. Although the two ends of the car were supported (with one end on jack stands and the other end setting on its truck bolster) no blocking or bracing was in place to support the car at its tack welded seam. On May 18th, 2011, an employee sustained fatal injuries when the tack welds failed on the car he was working under allowing the car to fall. One feasible and acceptable method to correct this hazard is to place shoring, blocking, or bracing materials (such as appropriately rated screw jacks) underneath the car on each side of the seam to be welded to alleviate stress on the temporary welds and to ensure against downward movement of the car in the event the temporary welds fail. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
