Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by / crushing hazards: a)Work site at 220 Hall Street, Montgomery, Al: On or about April 27, 2011: The employer failed to ensure heavy equipment operators looked behind the equipment prior to traveling in reverse to prevent employees who may be positioned behind or near the equipment from being run-over. Feasible and acceptable abatement methods, among others, to correct this hazard include but are not limited to ensuring an effectively communicated work rule is established requiring heavy equipment operators to look behind their bulldozer prior to traveling in reverse in accordance with the Operator and Maintenance Manual for the Komatsu D41E,P-6 Bulldozer; pg 2-5 and pg 2-19.
