Text For Citation: 01 Item/Group: 002 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a struck-by and crushed by hazard: (a)On or about April 7, 2011 - in the Large Diameter Core room, the jib crane (underhung hoist) was not marked with the rated load. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers standard, ASME B30.16-2007, Section 16-1.1.1, Rated Load.
