Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: On or about August 3, 2011, an employee was exposed to struck-by hazards from a large, round hay bale. Employee was removing load-securement straps from a load of hay bales on the step deck semi tractor-trailer, which was parked alongside a ditch and leaning towards the ditch. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS: Develop a standard operating procedure for the loading and unloading of hay bales and tractor-trailer placement during removal of load-securement straps. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
