Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employees a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees, in that employees were exposed to the hazard of being struck by the load should the overhead hoist, a portion of monorail track system or the overhead crane fail: a) Maintenance Shop: On or about March 23, 2011 and at times prior, employees were exposed to the struck-by hazards in that the rated load capacity of the jib crane and the hoist capacity did not match. Employees using an underhung hoist that had a capacity of 1 ton and the overhead jib crane that supported the hoist, was only rated for 500 pounds. b) BlowMold; Plastic North Area: On or about March 23, 2011 and at times prior, employees were exposed to struck-by hazards in that the rated load capacity for the monorail system and the hoist capacity did not match. Employees were using an underhung hoist that had a capacity of 2 tons and the monorail was marked as having a load rating of 500 pounds. c) BlowMold; Plastic South Area: On or about March 23, 2011 and at times prior, employees were exposed to struck-by hazards in that the rated load capacity for the monorail system and the hoist capacity did not match. Employees were using an underhung hoist that had a capacity of 1 ton and the monorail was said to have a load rating of 500 pounds. Among other methods, feasible and acceptable abatement methods to correct this hazard is to ensure the supporting structure is designed to withstand the loads and forces imposed by the hoist. This is to be done in accordance to ASME/ANSI B30.16; Section 16-1.3.2, which states: "The supporting structure, including trolleys, monorail, or crane, shall be designed to withstand the loads and forces imposed by the hoist for the rated load." d) BlowMold; Plastic South Area: On or about March 23, 2011 and at times prior, employees were exposed to the hazard of being struck in that the rated load capacity of the monorail track system was not marked. A hoist with a rated load capacity of 1 ton was attached to the monorail. Among other methods, a feasible and acceptable abatement method to correct this hazard is to mark the rated load capacity for monorails in accordance with ANSI B30.11b-1990; Section 11-1.1.2.
