29 CFR 1910.119(e)(5) The employer failed to establish a system to promptly address the PHA recommendations ; assure that the recommendations are resolved in a timely manner and that the resolution is documented. The employer violated this standard on or about June 21, 2011 when a review of the P&IDs and 2010 PHA report indicated that PHA recommendation item No S-5008 to add a note to the bypass line of safety valve, SV-020l081, in red-lined P&ID No: A-26116-7-0130-702 was not complete. Pursuant to 29 CFR 1903.19(d), within 10 days, the employer must submit an abatement plan describing the actions it is taking to ensure that PHA recommendations are completed in a timely manner. The violation must be abated no later than 10 days from the receipt of this citation.
