Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injury: a) 939 Cedar Lane - On or about September 11, 2010, an employee was unjamming the hydraulics on the rear of a front loading dump truck and was using a piece of wood (36 inches long and 2 3/4 inches thick) to support the weight of the tail gate, when the wood dislodge and caused fatal injuries. ABATEMENT NOTE: Among other methods, a feasible and acceptable means of abatement would be to comply with ANSI Z245.1-2007 entitled "Mobile Wastes and Recyclable Materials Collection, Transportation, and Compaction Equipment Safety Requirements," Section 7.2.2 "Support Points," which states that components that must be raised for service or maintenance shall be provided with attachment points, or have attachment points identified, for additional support. No abatement certification or documentation required.
