Text For Citation: 02 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a fire and explosion hazard. (a)On or about May 05, 2010 - AP Recovery Pilot Plant, Building 7352, Test Area 10, Redstone Arsenal, AL, the employer did not verify the suitability of equipment to do the work of processing an ammonium perchlorate (AP) and n- Butanol slurry. The employer utilized a (horizontal screw type) decanter centrifuge that was not documented to be suitable for use in an environment containing flammable and/or explosive dusts and flammable vapors to process energetic material. Among other methods, one feasible and useful method to correct the hazard - The hazard could be abated by following the requirement in the US Army's Standing Operating Procedures for Energetic Processing Module (EPM) Prototype Ammonium Perchlorate Recovery dated March 2, 2009 (SOP RA-0000-P-WD- GM-064), which was stated that any changes in equipment require a review by the Hazard Analysis Working Group (HAWG) and a revised SOP.
