Text For Citation: 01 Item/Group: 001A Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling: a)On or about 2/14/11, Twin West Plains: The top of a four foot fiberglass step ladder was used as a step. b)On or about 2/14/11, Twin West Plains: A four foot fiberglass step ladder was in use with a damaged top and damaged cross bracing. Among other methods, feasible and acceptable abatement methods to correct these hazards are to: 1) Follow the requirements of the American National Standard for Ladders - Portable Reinforced Plastic - Safety Requirements, ANSI A14.5-2000 edition. Including, but not limited to: Section 9.3.2(1) which requires that the top step and top cap not be used as step. Section 9.4.2 which requires that broken or bent ladders shall be marked and taken out of service.
