Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazards: a) On or about 02/28/11, and at times prior there to, an employee was exposed to the hazard of falling while using a 12 foot fiberglass step ladder in folded state. The employee was exposed to a fall of approximately 11' 6" to the concrete ground below. The ladder was leaning on the lip/ frame of the open hinged hatch door. b) On or about 02/28/11, and at times prior there to, an employee was exposed to the hazard of falling while using a 12 foot fiberglass step ladder in folded state. The employee was exposed to a fall of approximately 11' 6" to the concrete ground below. The step ladder was leaning on the lip/ frame of a open hinged hatch door. While using the ladder to access the 11' 6"ceiling catwalk, the employee used the top cap and top step of the step ladder. Among other methods, a feasible and acceptable abatement is to use the ladder in accordance with provisions stipulated in ANSI/A14.5-1992 for Ladders--Portable Reinforced Plastic, Paragraph 9.3.1.2, 9.3.2.1 as applicable. Essential Elements include, but are not limited to, the following: 1. Step ladders shall be used only in a fully-open position.2. Users shall climb or step no higher then the step or rung indicated on the label marking the highest standing level on a stepladder. 3. The step ladder base shall be placed with a secure footing on a firm, level support surface. 4. Users shall ensure that the step ladder is fully opened, with spreaders locked, and all feet contacting a firm level support surface.
