Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to the hazard of a falling and/or being struck by a mulch hopper that was not adequately secured to the lifting carriage and or forks of the Caterpillar 938F Wheel Loader: (a)1402 Eshelman Mill Road, Willow Street, PA - A chain used to lift the mulch hopper was not adequately secured to the forks of the Caterpillar 938F, thereby exposing employee(s) to being struck by the mulch hopper, on or about March 2, 2011. Among other methods, one feasible and acceptable method to correct this hazard is to provide and adequately secure equipment being lifted on the forks, in accordance with ANSI/ASAE S355.4 (2010), Section A.17 and A.18 or utilize equipment such as a lift jib.
