Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush hazards and struck-by hazards during the operation of powered industrial trucks which did not have restraint systems (seatbelts) installed. a) Throughout the facility - on or about 10/28/10 employees operated the Nissan type L forklift (LPG powered) and the Hyster forklift (LPG-powered after conversion) for loading and unloading materials in the Shop and in the Warehouse. Neither of the two vehicles had seatbelts installed. Abatement method: Among other methods, one feasible and acceptable abatement method to correct this hazard is to install and utilize a restraint system in accordance with the current version of the American National Standards Institute (ANSI)/Industrial Truck Standards Development Foundation (ITSDF) B56.1-2009, "Safety Standard for Low Lift and High Lift Trucks". In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
