Text For Citation: 03 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that foundry employees working with Tube Shake-out and Shake-out cranes were exposed to the potential hazards of being struck-by or crushed by materials carried by the cranes if the materials were to be disengaged from the crane hooks due to missing safety latches. a)Kenton Iron Products LLC, Kenton, Ohio: On or about April 28, 2010, the employer did not ensure that the hook for the Shake-out crane, a CM-2 Ton crane, located in the foundry area was equipped with a safety latch, exposing employees to struck-by and crushing hazards should the object being lifted roll out of the hook. b)Kenton Iron Products LLC, Kenton, Ohio: On or about April 28, 2010, the employer did not ensure that the hook for the Tube Shake-out crane, a Budgit manufactured Series 6000, Model BAH 0130, located in the foundry area was equipped with a properly working safety latch, exposing employees to struck-by and crushing hazards should the object being lifted roll out of the hook. Among other methods, one feasible and acceptable method to correct this hazard is to comply with the requirements of ASME B30.10-2009 "HOOKS Safety Standard for Cableways, Cranes, Derricks, Hoists, Jacks, and Slings" and ASME B30.16-2007 "Overhead Hoists (Underhung)". This employer has received a citation for this standard or a substantially similar standard as reflected in inspection #311604938, Citation #02, Item #01, issued on January 23, 2009 and became a final order on March 10, 2009. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation ( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
