Text For Citation: 01 Item/Group: 003 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees working in the foundry were exposed to potential struck by and crushing hazards do to the inadvertent overloading of molder spreader bars due to the lack of rated capacity marking on these lifting devices. a)Kenton Iron Products LLC, Kenton, Ohio: On or about January 19, 2010, the employer did not ensure the spreader bar used at the piston molding station was marked with its rated load capacity, exposing employees to struck-by and crushing injuries or death should the spreader bar become overloaded, causing it to fail and drop its 1,000 lb load. b)Kenton Iron Products LLC, Kenton, Ohio: On or about January 19, 2010, the employer did not ensure the spreader bar used at the Rotolift molding station was marked with its rated load capacity, exposing employees to struck-by and crushing injuries or death should the spreader bar become overloaded, causing it to fail and drop its 900 lb load. Among other methods, one feasible and acceptable method to correct this hazard is to ensure the under the hook lifting devices are labeled according to ANSI B30.20 Below the Hook Lifting Devices. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation ( using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
