Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by/crush-by hazard: 1) On or about 04/27/2009, at 7250 South Dadeland Blvd., in the city of Miami, FL, employees operating a truck mounted concrete pump were exposed to the hazard of being struck by/crush by a boom concrete pump that had two suspended end rubber hoses of approximately 7 meters that exceeded the maximum length required by the manufacturer (4 meters). 2) On or about 04/27/2009, at 7250 South Dadeland Blvd., in the city of Miami, FL, it was determined that employees operating a truck mounted concrete pump were exposed to the hazard of being struck by/crush by a boom concrete pump that was used to lift or drag weights (power trowels) with the boom. Among other methods of abatement, one feasible and acceptable method to correct the hazard is to operate the truck mounted concrete pump in accordance with the recommendations outlined in the Manufacturer's Use Handbook and American Concrete Pumping Association by: 1) Not exceeding the maximum total weight for system hanging on the boom (Max. length end rubber hose 4 meters), and 2) Prohibiting the employees to lift or drag weights with the boom. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
