Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about and times prior to February 17, 2009 employees were exposed to caught between and struck by hazards created by the uncontrolled upward looping of unspooling coiled steel while threading the feeder for the 150 ton Aida mechanical power press at work station #1201. Feasible abatement methods would include but not be limited to: following manufacturer's recommendations for the safe operation and settings on the Perfecto Coil Feed machinery, Installation of emergency stop buttons, installation of an upper limit to control undesired looping and compliance with ANSI B11.18-1997 Safety Requirements for Construction, Care and Use of Machines and Machinery Systems for Processing Strip, Sheet or Plate from Coiled Configurations.
