Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to Struck by and crushing hazards from unstable and falling bridge pilings: a) On 8/18/09, eight (8) 14" diameter steel bridge pilings of 28 to 36' lengths were standing on end in a template waiting to be capped and driven in the Sawyer Creek bed adjacent to Koeller St. in Oshkosh, WI. Neither these pilings, or the ten (10) foot high by 60 foot length template were secured to the ground in any way, or driven to prevent lateral movement of the template and pilings. While two (2) workers were attempting to align the farthest west piling prior to capping with a crane operator, (from which the driving rig was suspended) the template and pilings shifted and fell to the west. Feasible and acceptable means to abate this hazard would include: 1) To secure the template to the ground prior to placing pilings into the template. 2) To drive the pilings one at a time, and only place the next piling when the previous one has been securely driven. 3) To drive several pilings in order to secure the template prior to placing the rest in the template. 4) To pre-drill starter holes for the pilings. 6) Where the template is used to structurally support the pilings, an engineering survey should be performed to ensure the safety and stability of the template and pilings.
