Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard. a) The employer did not provide and require the use of a reflective vest and/or clothing for employees who walk through the yard in order to make employees more visible by heavy equipment operators on or about November 26, 2008. Feasible and acceptable abatement methods include, but are not limited to the following: 1) Provide and require the use of reflective vests and/or clothing. 2) Provide training to all employees on the danger area around equipment which includes a circle of 23 feet as defined in the Volvo L90E/L110E/L120E Operators Manual. 3) Ensure that safety equipment installed on the Volvo Loaders remains functional and when a defect is discovered it is either repaired or the machine taken out of service.
