29 CFR 1910.307(c)(1) Electrical installations. Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Requirements for each of these options are as follows: Intrinsically safe. Equipment and associated wiring approved as intrinsically safe is permitted in any hazardous (classified) location for which it is approved; Note (c)(2)(i): NFPA 70, the National Electrical Code, lists or defines hazardous gases, vapors, and dusts by "Groups" characterized by their ignitable or combustible properties. A. The employer did not ensure that equipment, wiring methods, and installations of equipment in hazardous (classified) locations were intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. This violation was identified during inspections of the facility from November 2009 to March 2010, Silver Eagle Refining Inc., 2355 South 1100 West, Woods Cross, UT, 84087. Compliance Officers observed an exposed light bulb in the railway load out shack that was not for the class of location and for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that will be present. Per Silver Eagle site electrical classification plans, this area is classified as a Class 1, Division 2 area. The light fixture was missing the approved hazardous explosion cover and did not meet the requirements of NFPA 70. As a result, employees were exposed to potential serious injury or death should the wiring ignite flammable vapors. This may be a system-wide occurrence that requires evaluation of all electrical wiring throughout the facility. B. The employer did not ensure that equipment, wiring methods, and installations of equipment in hazardous (classified) locations were intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. As seen in the lab, was exposed wiring not enclosed in an approved hazardous explosion cover and did not meet the requirements of NFPA 70. As a result, employees were exposed to potential serious injury or death should the wiring ignite flammable vapors. This may be a system-wide occurrence that requires evaluation of all electrical wiring throughout the facility.
