North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to steam hazards when not provided with written operation and maintenance procedures: a)establishment, Levine Science Research Center, mechanical room C04 - where employees were not provided with operation and maintenance procedures for the steam pipe system when required to work with steam lines. On or about May 14, 2008, an employee was fatally injured when starting up the steam line system when the high pressure (> 15 PSIG) steam line ruptured. Abatement note: One feasible and acceptable abatement method is to develop written operation and maintenance procedures for piping systems deemed necessary to ensure safe facility operations.
