Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fire and explosion: a/ J.W. Goodliffe/Linden NJ Site - Main Bldg. Dock/Yard Area: Employees are exposed to the hazard of fire/explosion resulting from the use, storage and handling of compressed gases. Gases include but not limited to: Oxygen, Nitrogen, CO2, Argon, Acetylene, Propane. At the time of this inspection the employer could not provide documented information to confirm that procedures were in place to ensure that all employees involved in the compressed gas cylinder filling operation(s) were performing the operation(s) adequately, safely and uniformly. Documented procedures (to include but not limited to) how the below mentioned are to be performed: 1/ Checking compressed gas cylinders - proper, fixed and legible labels; retest dates; defects; arc burns; need for repair; etc. 2/ Cleaning - ensure compressed gas cylinders/valves are free of oil, grease, debris, welding slag, etc. Ensure product used in cleaning operation does not create an additional hazard(s). 3/ Check Valves - connection threads for damage and engagement 4/ Check pressure relief devices - proper pressure and damage 5/ Leak test using leak test solution 6/ "Sniff" test to check for contaminants 7/ "Dead Ring" test to check for interior corrosion 8/ Vacuum/blowdown - ensure cylinders are free of product/contamination 9/ Filling cylinders - proper capacity/pressure 10/ Cylinder Caps - ensure properly positioned 11/ Handling full/empty compressed gas cylinders - lifting procedures 12/ And/or other approved procedures 13/ Training in the documented procedure ABATEMENT: Among other methods, one feasible and acceptable means of abatement would Part 173,304 Filling of Cylinders with Liquefied Compressed Gas.
