Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to the employees in that employees were exposed to struck by hazards: a.Route 17 By-pass, Stollings, WV - An employee was adjusting the gooseneck of the Talbert trailer when the shim/adjustment block shot out from the trailer. The position of the gooseneck stop pins were not verified prior to adjustment of the gooseneck to prevent parts from being ejected from the trailer, as determined on 03/28/08. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard would be to establish and to enforce accepted safe practices such as but not limited to a) develop procedures to verify gooseneck stop pins are in the retracted position prior to operating the gooseneck controls, b) verify that both the left and right side gooseneck stop pins are either retracted or extended prior to applying weight to or releasing weight from the gooseneck, c) ensure ejection path of shim/adjustment blocks are clear before releasing or actuating the gooseneck controls, d) use another type of trailer that does not bottom out when crossing the railroad crossing.
