Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to hazards such as, but not limited to, explosions, struck-by and steam hazards due to the lack of proof testing prior to placing the tire curing unit into service: (a) Titan Tire Corporation of Bryan, Bryan, Ohio: on or about November 23, 2008, the employer did not ensure that proof testing for maximum allowable working pressure was conducted on the 53/80R-63 tire curing unit 414 prior to placing it in service. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. Among other methods, one feasible means of abatement to correct this hazard is to ensure the mechanical integrity of the unit before operation by conducting a hydrostatic test after fabrication and examinations have been completed, test pressure at every point being equal to at least 1.3 times the maximum allowable working pressure, and all loadings that may exist during this test being given consideration.
