Text For Citation: 01 Item/Group: 001 Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of fire and/or explosion: a.#6 Boiler - Boiler doors were secured by placing rocks onto the latches. b.#6 Boiler - Boiler doors were not properly secured against escaping embers/fire. c.#5 Boiler - Boiler doors to the boiler were bulged and cracked, allowing escape of embers/fire. d.#5 Boiler - The steam tanks were not properly sealed against the boiler walls, allowing the escape of embers. ABATEMENT NOTE: Among other methods, feasible and acceptable means of abatement are to: a) Properly install and maintain doors on the boilers such that the burner and fired areas of the boiler are isolated. b) Seal boilers so that the boilers are secured against escaping fire and/or fugitive embers.e
