Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: On July 13, 2007, and at times prior thereto, at the West Lehman Field on the Gardner-Denver mobile drilling rig, the wire rope was not routinely inspected, nor used and installed properly as it had the following visible deficiencies, such as, but not limited to: a.the Crosby clips used to create an eye were installed incorrectly by placing the saddle on the dead end of the wire rope. b.the Crosby clips used to create an eye made a large loop instead of using a thimble with the wire rope wrapped around it. c.The Crosby clips were not installed one base width from the dead end. d.The wire rope on the drum had a broken or cut strand within one lay of the rope. Among other methods, one feasible and acceptable abatement method involves the routine inspection and installation of wire rope terminations according to The Crosby Group, Inc.'s recommended guidelines.
