Text For Citation: 01 Item/Group: 003 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause dealth or serious physical harm to employees in that employees were exposed to falls from height: Each employee working on a tank truck roof with unprotected sides and edges, is not protected from falling by a personal fall arrest system (PFAS) or an appropriate custom-fit guardrail system. This hazard occurred on or about September 25, 2007, and at times prior thereto at the Asphalt Loading Rack and JP-8 Loading Rack, where process operators and contract/visiting product truck operators were not protected from fall hazards of 10 feet to the ground. One feasible means of abatement for this hazard, among others, would be to ensure employees performing loading work are protected from falls, and that no employee is allowed onto a vehicle deck/platform more than four feet high without adequate fall protection.
