Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to employees in that employees are exposed to fire hazards: The employer does not control ignition sources through proper bonding during manual and open- system mixing operations with flammable liquids. This hazard occurred on or about September 25, 2007 in the FCC Unit complex, where the employer failed to ensure proper bonding during mixing operations between the pour spout and day pots to mix raw naphtha (flammable liquid) with EC-1013-A corrosion inhibitor chemical for thinning purposes. One feasible means of abatement for this hazard, among others, would be to ensure proper grounding and bonding of containers is done to ensure static discharge is eliminated.
