Text For Citation: 01 Item/Group: 002 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between hazards and hazards related to hi-pressure caused by misuse of equipment: In the heavy equipment maintenance shop, on or about October 13, 2006, and on dates prior thereto, employees were using a Porta Power unit with shop-made appurtenances for bearing/bushing insertion into heavy equipment. The employer did not ensure employees were trained in the safe operation of the hydraulic equipment as well as the proper selection of associated parts for use with hi- pressure hydraulics to prevent a mishap. Feasible methods to abate this hazard include,but are not limited to the following: The employer must ensure that all operators are trained in the hazards associated with the safe operation of the Porta Power unit as required by ANSI B30.1 and the manufacturer's operating instructions for hydraulic cylinders.
