Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Public Law 91-596, Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard from a pressurized vessel. On or about 8/24/07, an employee was struck by the door of a pressurized vessel (autoclave) used for tire re-treading in a vulcanizing process inside a pole barn behind the tire shop. The Bandag pressure vessel was not inspected by a qualified inspector at the time of installation. The pressure vessel was not verified for safe operation prior to its use by the employer. Among other methods, one feasible and acceptable abatement method to correct this hazard is to have the pressure vessel inspected by a qualified inspector familiar with ASME Boiler and Pressure Vessel Codes and API 510.
