Text For Citation: 01 Item/Group: 003 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from an elevation: On or about October 16, 2006, and at times prior and after thereto, employees were exposed to the hazards of falling approximately 28 feet to the level below when exiting and reentering the personnel platform attached to the Nissan forklift, Serial No. 9C2206 while attempting to free jammed pallets. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use a piece of equipment designed to lift personnel to another platform or level equipped with a gate or door for safely entering and exiting as required by ANSI/ITSDF B56.1-2005, Sections 4.17.3(p) & (r) which prohibits using a platform attached to a forklift for entering and exiting except at ground level, and requires personnel to remain on the platform while in the basket.
