Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Title 40.1-51.1.(a); Code of Virginia: 7222 Janet Pl, Falls Church- where employees were performing tree removal operations. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to approximately 45 foot fall hazards while operating a GMC 7000 Bucket Truck with a Model LR-50 bucket/boom manufactured by Asplundh Manufacturing Division, to lower a severed tree branch to the ground. Placards mounted on the side of the truck indicated that the maximum bucket weight capacity was 300 pounds, with no weight to be attached to the boom. On August 2, 2006, the operator of the aerial lift was using the bucket/boom to assist with the lowering of a large tree branch to the ground. The branch had been cut from the tree and was suspended in the air by a rope attached to the tree and was hanging over the aerial lift truck. The victim apparently unhooked his lanyard from the aerial lift bucket and connected the lanyard to the ropes so that he could use the aerial lift to pull the branch away from hanging directly over rear of truck. When he moved the boom to pull the branch sideways, the bucket detached from the boom causing the victim to tip out of the bucket and fall approximately 45 feet on to the top of the truck. One feasible and acceptable abatement method, among others, to correct this hazard is for the employer to assure that its aerial lift operators are trained on, and comply with, the following manufacturer's operation and use instructions contained within warning placards on the truck: "Maximum of 300 lbs. allowed in basket(s). Additional weight of 0 lbs. must be attached to the upper boom or jib boom." "Do not load beyond rated capacity."
