Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: At 5301 Polk St #20, Houston, TX. 77023 on or about 08 March 2007, the employer failed to inspect the pressure vessel (rubber curing autoclave) manufactured by Gray Welding on an annual basis and or after the pressure vessel was temporary out of service and put back in service. The hazard is exposure to explosion by death and or injury due to the lack of inspection methods on an annual basis but not limited to when the pressure vessel was decommissioned and then put back in service. Among other methods,one feasible and acceptable abatement method is to: Comply with the General Requirements for Inservice Inspection of Pressure-retaining Items RB-1000 of the National Board Inspection Code are implemented in the workplace pertaining to the pressure vessel (rubber curing autoclave). The National Board Inspection Code RB-2000 Personal Safety and Inspection Activities with reference to RB-2100 Personal Safety states the owner shall implement accepted engineering safety procedures satisfactory to the inspector. ***Certification AND Documentation of Abatement Required*** DISCLAIMERS: 1) You are not limited to the abatement methods suggested above. 2) The methods explained are general and may not be effective in all cases. 3) The employer is responsible for selection and carrying out an appropriate abatement method.
