Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by equipment: On or about February 15, 2007, the employer did not ensure the Foremost Mobile Rig B-47, Serial #90125, was maintained in a a safe operating condition. Defects noted included but are not limited to: a) Foremost (Mobile Drill) safety decal was missing back by operator control panel; b) The glass is broken on the hours operated gauge; c) The passenger side vehicle (in the back), reflective light is broken; d) The driver side tool box, the door is being held on by a bungee cord; e) There is a loose bolt in one of the openings were the operator stands in the back. Among other abatement methods, useful and feasible, one method to abate this hazard would be to comply with the recommended guidance in the Manual for Operations / Parts / Maintenance for Mobile Drill B-47 or to comply with the recommendations given by the National Drilling Association, Drilling Safety Guide revised 2005, section 3 and ensure that the Drill Rig is being inspected, repaired, maintained, and functioning properly.
