Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a falling load, or material, due to failure of rigging hardware during the lift: a.)Motors & Controls Corporation, Langley Air Force Base (LAFB), Nealy Avenue and LaSalle Avenue, Vehicle Blocker #5. On or about March 1, 2008, employees working with, and in the vicinity of loads suspended beneath the fork tines of the JCB Forklift Truck, were exposed to the hazard of falling loads, in that safe rigging methods were not used to lift the rising vehicle blocker due to the eye bolts not being properly positioned to prevent an angular pull during the lift. This resulted in the failure of an eye bolt. ABATEMENT NOTE: Among others, an acceptable and feasible abatement method for this hazard is to utilize a spreader bar when making a lift of this type.
