Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employee(s) were exposed to struck by and crush hazards while operating equipment: a) On or about July 19, 2006, and times prior thereto, an employee was exposed to struck by and crushing hazards, when operating a John Deere AMT 626 vehicle and safe operating conditions were not maintained. The operator was unable to maintain control of the vehicle during operations on a steep hill located down hill from Flagger Stand #6. Among other methods, feasible and acceptable means to correct this hazard include, but are not limited to: 1. Obtain and provide vehicle reference manuals for employees tasked with operating respective vehicle. 2. Operate vehicle in accordance to the manufacturer's recommendations. 3. Before allowing employees to operate vehicles on premises, including John Deere AMT 626 vehicle, ensure that they have successfully completed training in a language they can understand consisting of a combination of formal instruction, practical training and evaluation of the operator's performance at the workplace.
