Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by injuries, in that, the operator of the rough terrain forklift did not lower the load-engaging means fully prior to leaving the operator's position during bag house hopper assembly activities: a) Irondale Industrial Contractors, Inc., 6767 County Road 9, Delta, OH: On or about April 27, 2006, employees were exposed to struck-by injuries, in that, the operator of the rough terrain forklift (Lull 844C) did not lower the load-engaging means fully prior to leaving the operator's position during bag house hopper assembly activities. The hopper panel that was rigged to the 70 ton crane (Link Belt AC 70) and shackled to a rough terrain forklift was displaced after an employee removed the shackling. This employee suffered fatal head injuries as a result of his fall through the hopper panel's hatch opening. No additional abatement information is required for this item.
