Text For Citation: 01 Item/Group: 001 Hazard: ELECTRIC
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and or an explosion: a) Production Area - An employee operating the Walter CNC machine #1124 was burned when he opened the front door of the machine to investigate smoke coming from inside the machine during a production cycle. A interlock safety device had failed to secure the front door in the closed position and allowed it to open slightly which stopped the flow of coolant to the part. The part had overheated the coolant causing the vapor to ignite when the operator opened the front door resulting in an explosion and fire.
