Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: fall and/or crushing hazards: (a) The 5000 lb Heli powered industrial truck, model #FD50, serial #30390, 8000 lb Toyota powered industrial truck, model #02-FDE35, serial #FD45-11194 and the 5000 lb Toyota powered industrial truck, model #02-4FD25, serial #4FD25-28309 seat belts were removed when the seats were replaced with automotive seats. Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with ANSI B56.1 consensus standard for powered industrial trucks; ASME/ANSI b56. 1a-1989 Addenda to ASME/ANSI B56.1-1988, and ASME B56.11993 Safety Standard for Low Lift and High Lift Trucks that require the use of seat belts. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
