Text For Citation: 01 Item/Group: 003 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of serious crushing injuries: a)On or about July 27, 2006, an Orion bus (RTD 2252X, Colorado License Plate 536- MCP) was towed in an inappropriate manner, in that, the underlift was not connected to appropriate tow points. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to follow the instructions detailed in the Orion Manual for Jacking and Towing Safety. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
