Text For Citation: 01 Item/Group: 004A Hazard: BURNS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Fire/Explosion Hazards a) Worksite located at the Alloy Building, 9 Roy Street, Dover, NJ Employees sustained severe burns due to the explosion/flash fire in the "C" furnace due to the pouring of molten metal. Employees involved did not receive regular training in reference to the hazards associated with working on or near the equipment. The "C" furnace was not equipped with an audible ground-fault alarm nor proper pressure relief system on the melting chamber. Violation observed on or about 7/25/05 FEASIBLE AND ACCEPTABLE METHODS OF ABATEMENT, AMONG OTHERS, TO CORRECT THIS HAZARD ARE: Follow recommendations in accordance to the 1985 NFPA 86D, Industrial Furnaces Using Vacuum as an Atmosphere: a) Section 9-5.7, Ground fault detection system that sounds an alarm whenever molten metal gets close to the water cooling lines near the induction coils. b) Section 5-3.4, Pressure relief valve on the vacuum chamber that does not release excess pressure on the melters' platform exposing employees. c) Section 1-9(c), Provide regular retraining of regular vacuum induction furnace operators in areas such as, but not limited to, those listed in Section 1-9(f).
