Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught in blow mold machinery if a safeguard (safety feature) failed to operate: (a)On or about June 23, 2006, blow mold department, the employer did not establish and follow an effective program for periodic and regular inspections for extrusion blow molding machines to ensure that safeguards were in safe operating condition. Among other methods, one feasible and reasonable method to abate this condition is to provide a documented inspection of the equipment in accordance with ANSI B151.21. *** Certification and Documentation of Abatement Required ***
