Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: On March 21, 2005 and at other times prior thereto, at this job site, employees required to use an aerial lift device mounted on the bed of a Ford truck were exposed to the hazard of being struck by the vehicle. Defective equipment on the Ford High Ranger aerial lift included items, such as, but not limited to, the following: a.Emergency brake not connected b.Electrical wires connections are taped and dangling a few inches from floor board c.Modified oil pressure gauge hose/line blocking access to the emergency brake release d.Hydraulic hoses worn brittle and leaking Among other methods, one feasible and acceptable method to correct this hazard would be to inspect and repair defective equipment prior to vehicle use; comply with Department of Transportation; Federal Motor Carrier Safety Regulations.
