Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The Employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) On or about February 4, 2005 employees utilizing a VAC/ALL catch basin & jetter pressure equipment while performing bond release operations in a 48" storm drain pipe were exposed to the hazards associated with being struck by pressurized water jetting hose and nozzle, as a result of utilizing the jet hose/nozzle in a hand held manner without secure mechanical restraint or equivalent. Among other methods one feasible and acceptable method to correct this hazard would be to implement procedures to ensure that the manufacturer's specifications, as contained in the VAC/ALL by Leach operator's manual, be followed, including the use of equipment for appropriate pipe size applications, prohibition of use of equipment for hand held jetting hose/nozzle, mechanically restraining jetting hose/nozzles, utilizing appropriate nozzle design applications, and proper coupler sled use.
