Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not provide a place of employment that was free from recognized hazards that were likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using a drill rig service winch with a deficient wire rope end termination. 696 W. North Avenue jobsite - The U-bolt wire rope clip end termination for the wire rope on the drill rig service winch did not have an adequate amount of turn back of wire rope. Among feasible means of abatement would be to comply with the Wire Rope Technical Board's "Wire Rope Users Manual" and the SoilMec R-312/200 Hydraulic Drilling Rig Operation and Maintenance Manual regarding specifications for making wire rope end terminations.
