Text For Citation: 01 Item/Group: 001 Hazard: CAUGHT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught in an in-running nip/pinch points on a moving belt conveyor and were unable to shut the conveyor off quickly because no emergency stop pull cord or similar device was present: a) On or about 09/22/04, in the Veneer Dept.: An employee's hand got caught in an in- going nip point at the "turnaround roll" of the mylar belt conveyor and his arm was amputated when it got caught in the nip point at the bottom chain and sprocket wheel of the drive system for the mylar belt conveyor at the exit end of the Italpresse Hot Oil Veneer Press. Among others, a feasable method to eliminate or materially reduce this hazard is to equip the mylar belt conveyor with permanently installed pull cords or similar stop controllers which comply with ASME B20.1-1996, Safety Standard For Conveyors and Related Equipment, Section 5.11.2(c) and Section 6.1.2(c).
