Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fall from elevation and stuck by rotating objects: (a) On or about 06/13/05, Ringo Drilling Rig #2 was being taken apart for transport to a new location. A gin pole truck having a wire rope secured to a tail chain by a knot was being used to move the rig. A swamper attached the tail chain to a Kelly and signaled the gin pole truck driver to lift it. The tail chain slid up the Kelly pipe causing it to rotate the end being held by the swamper to quickly rise taking the swamper with it. The swamper fell approximately 21 feet (6.4 m) landing on his head. One feasible and acceptable abatement method, among other, to correct these hazards is: 1. "A tagline or restraining device shall be used to control the load." Ref: pg V-2, Halliburton Energy Services Group Safety and Health manual, 1992. 2.
