Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY THE LOAD OR LOADER: a)Asphalt Mixing Silo # 2, on or about 08/11/2004: A front end loader was being used for work in a manner other than specified by the manufacturer in that an 'I' Beam was placed inside the bucket and chained in a vertical position and then used to lift and poke a hardened accumulation of asphalt mix in the bottom of a silo. Among other methods one feasible and acceptable method to correct the struck by hazard is to: Ensure employees never attempt to put the front end loader to uses other than that specified by the manufacturer's manual, Page 47, 48, & 49, Kawasaki Wheel Loader Operation & Maintenance Manual, Model 85ZII. b)Asphalt Mixing Silo # 2, on or about 08/11/2004: The operator of the front end loader did not ensure the area in the front of the loader was clear of personnel and obstructions before moving the front end loader. Among other methods one feasible and acceptable method to correct the struck-by hazard is to: Ensure the area is clear of personnel and obstruction prior to moving the loader per the manufacturer's manual, Page 43, Kawasaki Wheel Loader Operation & Maintenance Manual, Model 85ZII. Abatement documentation must be submitted for these items.
