Text For Citation: 01 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a)On or about 8/26/2004, 2nd floor housing, employees exposed to a fall to the ground floor below when the gate across the opening of the freight elevator hoistway is open with no means available to secure it in place when the elevator is not there as required in ASME 17.1-1996. A feasible means of abatement is to provide interlocks or mechanical locks and electric contacts on the hoistway door opening as required in ASME 17.1-1996, par. 111.b and which is cross referenced in ASME 17.1-2000, par.2.12.1. The design requirements for interlocks is referenced in ASME 17.1-2000 paragraph 2.12.2.4, and the mechanical locks and elelctric contacts, if permitted, are referenced in paragraph 2.12.2.3. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
