Text For Citation: 01 Item/Group: 001 Hazard: CUTS
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards: (a)On or about 3/3/04, employees were exposed to the hazard of amputations because the top loading door safety switch on the paper bailer was inoperative permitting the operation of the bailer with the top loading door in the open position which exposes employees to amputation hazard when the baler ram is actuated. Among others, one feasible and acceptable abatement method to correct this hazard is to comply with The American National Standards Institute, ANSI Z245.5-1982, Safety Requirements for Baling Equipment, Section 4.2.2.1 and Section 4.2.2.2: In that the machine shall have a chamber loading door that completely encloses the loading chamber before the ram can be activated into its compression (downward) stroke. OR, IN THE ALTERNATIVE 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a)Employees exposed to amputation hazards during downward compression cycles of horizontal baler which could be operated with the top loading door open due to a defective safety interlock switch.ck
